Servicing For Adults and Children
Community Habilitation consists of services designed to assist individuals in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to reside successfully in their home and engage in community based activities. Our Direct Support Professionals provide this service as one to one support so that our individuals can achieve their valued outcomes to increase their independence.
Our Direct Support Professionals:
- Work with individuals on a one-to-one basis
- Help Individuals achieve the valued outcomes stated in his or her Life plan and/or Staff Action Plan
- Increase independence
- Promote Self Esteem
- Engage in various activities in the community
- Promote positive communication
- Develop trust and meaningful relationships
Examples of Some Valued Outcomes:
- Independence
- Reading Comprehension
- Completing homework assignments
- Enhancing social skills
- Improving activities of daily living skills
- Accompanying individuals to various places in their communities for recreation and other activities
- Using technology to enhance community skills
- Managing/Budgeting/Money Skills
- Learning how to travel using public transportation
- Learning pedestrian safety while traveling in the community
- Promoting good health through exercise and nutrition
- Learning shopping skills
- Safety skills
